Invitation to join the PCN 

The Kawartha Lakes Haliburton Primary Care Network (KLH-PCN) empowers you to shape the future of primary care in our region. By joining, you'll gain access to essential resources that help reduce administrative tasks, connect with Ontario Health Team (OHT) initiatives, and develop your leadership skills. You'll be part of a collaborative community dedicated to enhancing patient and provider experiences, influencing critical healthcare decisions, and tackling primary care challenges. Together, we can strengthen and improve primary care for everyone in Kawartha Lakes and Haliburton. 

You are eligible to join if you identify as a patient-facing primary care provider operating in the Kawartha Lakes and Haliburton County. 

Who can join? 

By joining KLH-PCN, members are expected to: ï»¿

  • Engage in at least one PCN initiative annually (e.g., working group, pilot project). 
  • Share feedback to help shape PCN priorities and services. 
  • Promote collaborative care and knowledge sharing within the network. 
  • Maintain up-to-date contact and practice information. 
  • Support the PCN’s mission to improve access, coordination, and quality of care.
