A key focus will be the emerging vision for locally connected community health hubs/medical neighbourhoods/medical homes — strengthening integration between primary care, community services, chronic disease management, mental health supports, prevention programming, and digital tools.

We want to hear directly from you regarding the opportunities, pressures, and priorities facing primary care, while also highlighting practical supports available to clinicians, including OCEAN eReferral/eConsult, the Centre for Effective Practice, Amplify, quality improvement resources, and community-based programs that can support patient care.
